Proactive Grants

studentsProactive grants are initiated by The Horizon Foundation to effect change in identified priority areas. Grants may be awarded for planning activities, piloting new health initiatives, developing innovative service models, and mobilizing various segments of the community to seek change. Grants may be solicited from a single organization or from a group of organizations. Examples of primary focus areas include current Horizon Foundation initiatives such as child and adolescent health and wellness, substance abuse, older adult health and emergency response.

Application Process

Proposals are developed at the invitation of The Horizon Foundation in collaboration with its staff, and may be submitted for review at any time during the year.
