The Horizon Foundation has demonstrated real leadership and commitment to a comprehensive and integrated approach to ensuring that Howard County, Maryland, will be prepared for the aging of its population. Their approach emphasizes the importance of building prevention into long-term care and ensuring that people have easy access to the services and supports they need to maintain their health and independence as they age.
